Abstract
According to BPJS Ketenagakerjaan in 2023, Indonesia experienced 265,334 workplace accidents between January and November 2022, marking a 13.26% rise from 234,270 cases in 2021. The construction sector was identified as one of the industries with a high accident rate. In this research, Fault Tree Analysis (FTA) was applied to examine the causes, focusing on finding the chain of factors that led to the accident. This research aims to identify the combination of contributing events behind workplace accidents in the Tawang-Ngalang Segment IV Road and Bridge Construction Project by utilizing the Fault Tree Analysis (FTA) method. The study identifies basic events from the top event, applies Boolean algebra to find the minimal cut set to determine the combination events that cause the top event, and then formulates risk control measures in compliance with hierarchical frameworks. The results of this study identified the primary root causes of the slope landslide in the Tawang-Ngalang Segment IV Road and Bridge Construction Project using the Fault Tree Analysis method. The main factors contributing to the landslide are outlined as follows. Insufficient competency and comprehension of workers in executing their assigned work. (1) Insufficient competency and comprehension of workers in executing their assigned work. (2) Poor supervision is attributed to the insufficient discipline and awareness exhibited by the workers, (3) Insufficient information outreach and communication across teams have caused workers to be uninformed about any changes that have been agreed upon. (4) Extreme weather conditions are compounded by inadequate soil stability.
