Abstract
The rate of work accidents in the mining industry is still relatively high despite the
implementation of various safety systems. Coal transportation activities with heavy equipment
and extreme road conditions are one of the main contributors to the risk of fatal accidents. This
study aims to identify and evaluate the risks that are classified as very high at PT. XYZ uses
the Hazard Identification, Risk Assessment, and Risk Control (HIRARC) method, and
formulates mitigation strategies in the form of preventive actions and contingency plans to
reduce risk levels and maintain the company's operational sustainability. Through risk
assessment using a matrix, 21 risk variables were found to be categorized as very high (red
zone), with the two main risks being slippery roads during rain and operators not knowing the
landslide zone. The cause analysis was conducted using a fishbone diagram, and then mitigation
measures were developed based on the risk control hierarchy. The results showed that the
integrated implementation of technical and administrative controls and the use of personal
protective equipment can significantly reduce potential hazards. This study concludes that
systematic risk management through the HIRARC approach is effective in mapping hazard
priorities and determining appropriate mitigation measures. The main recommendations are
aimed at improving engineering controls, operator training, and emergency response plan
readiness to prevent future work accidents.
